What you can do on this site:

Facilitate applications to treatment services for Status First Nations people
Apply for specialized treatment services - including complex, concurrent disorders (e.g. mental health and substance use, comorbidity)
Identify the need for long term treatments due to the complexity of a condition
Identify need for longer term care
Assess the availability of FNHA funded Treatment Centres for facilitating treatment.

This information will be used to determine eligibility for a subsidy.

Once you have submitted the application we will send you, as the referral partner, an email verifying that the application has been received. The application will be reviewed within 3 business days.
